To expedite your registration process, the following forms will need to be completed and returned to Jersey Elite on or before the first day of practice. Copies of all forms are also available at the gym. Parent information and welcome package will be provided to you upon registration:

- Jersey Elite Registration Form 10-11
- Medical Consent Form
- Jersey Elite Practice Gear
- A copy of your birth certificate (not original)
